Team A5 Ltd is a successful recruitment consultancy specialising in the temporary, permanent, and contract placement of high-caliber of candidates. We are a supported living provider, domiciliary care, and a home care provider. We are CQC registered for personal care in the United Kingdom.

We provide supported living for people living with learning disabilities, physical disabilities, autism, mental health, cognitive issues etc. We provide services for 18 and above, as well as 16 to 25 years. We work with complex cases.

Job Summary

We’re currently seeking an experienced Care Coordinator/Manager for our Supported Living with a strong background in delivering high-quality, person-centred care to individuals with diverse needs. Ideally, you’ll have prior experience in a similar role within the health and social care sector—preferably within Supported Care Living—and hold a Level 5 qualification (or higher) in Health & Social Care or an equivalent field. A solid understanding of CQC regulations, safeguarding practices, and personalised care approaches is essential.

The ideal candidate will be highly organised, an effective communicator, and a confident leader. Flexibility is important, as the role may require occasional evening and weekend work. Proficiency in IT systems, making care support plans, making rotas, liaison with CQC and other health compliance, make use of Electronic Care Management platforms, is a must.

Service Coordination:

  • Act as the primary link between service users, their families, and support networks. Oversee the daily running of supported living services, ensuring smooth operations.
  • Ensure care delivery is tailored to the unique needs, preferences, and goals of each individual.
  • Match service users with appropriate support workers based on skills and compatibility.
  • Fully utilise the Electronic Care Management System for efficiency and quality service delivery.
  • Lead and guide staff in delivering person-centred support to adults with a range of needs, including learning disabilities and mental health challenges, promoting independence and personal development.
  • Develop in-depth knowledge of the organisation’s services and capabilities.
  • Demonstrate a flexible and proactive approach to the role, adapting as needed.
  • Commit to delivering exceptional care and support services.
  • Build collaborative, effective teams that make a positive impact.
  • Participate in the 24-hour on-call rota, ensuring continuous support for staff and service users.
  • Tackle challenges with a solutions-focused mindset to exceed service expectations.

Care Planning & Delivery:

  • Create, review, and update personalised care and support plans in collaboration with Trusted Assessors and Key Workers.
  • Carry out risk assessments and ensure appropriate safeguarding measures are implemented.
  • Work alongside multidisciplinary teams and external agencies to provide comprehensive support.

Audit & Compliance:

Experience in responding and resolving queries with Care Quality Commission (CQC), Quality Assurance (QA), Local Council, Government bodies and other healthcare compliance organisations.

Additional Requirements:

Applicants must have clean full UK driving licence and have access to their own vehicle.

Able to manage multiple locations. – Bexley, Medway, etc
