• This is an opportunity for a Senior Health Support Worker to enhance their knowledge and skills working with the Integrated Community Support Team.
• The Senior Health Support worker will deliver high-quality care in accordance with the guidance given and work delegated by registered nurses.
• The post holder will ensure a person-centred approach to care delivery which is safe and effective to ensure and maintain quality strategy.
• We are seeking an enthusiastic, dedicated individual who will have SVQ3 in healthcare, or working towards, or equivalent experience working within a caring environment/role.
• To actively participate in the Personal Development Plan process to fulfill the requirements of continuing professional development.
• Who will have the ability to work with people and as part of a multidisciplinary/agency team, also have the ability to work unsupervised.
• The ability to carry out assigned patient care tasks effectively within a busy environment by providing effective written and verbal communication skills

• A Full Valid UK Driving Licence Holder would be essential for this position.
